NP or PA - Primary Care (telehealth) [Supp.] Job

Employee and Community Health is seeking a Nurse Practitioner or Physician Assistant to work supplemental as part of a multidisciplinary care team utilizing telehealth monitoring and real-time telehealth sessions with patients with chronic conditions, such as but not limited to diabetes, CHF, and COPD. This position will involve, but is not limited to, providing comprehensive evaluation, chronic and acute management of chronic disease, physical assessment, ordering and interpreting diagnostic tests, prescription of pharmacologic agents and treatments, nonpharmacologic therapies, and counseling patients.

Department: Employee and Community Health/Primary Care Internal Medicine

Qualifications: Masters prepared graduate of an accredited school of nursing and Nurse Practitioner program. Current Minnesota RN license and certification or eligible to be certified as a FNP, ANP, or GNP (certification must be obtained within 6 months of employment date.) Or graduate of accredited Physician Assistant program with current PA certification by NCCPA and registration as a PA by the Minnesota State Board of Medical Practice. BLS certification required. DEA registration will be required. Minimum of two years outpatient primary care experience preferred. Please submit resume and 2-3 letters of reference.

Salary: Education, experience, and tenure may be considered along with internal equity when job offers are extended. The minimum pay rate is $37.60 per hour.
