NP or PA - Primary Care (Telehealth) Job

Employee and Community Health is seeking a Nurse Practitioner or Physician Assistant to work as part of a multidisciplinary primary care team. The NP or PA will provide comprehensive assessment, diagnosis, management and evaluation utilizing telehealth monitoring and real-time telehealth sessions with patients with chronic conditions. The provision of care includes but is not limited to comprehensive evaluation, ordering and interpreting diagnostic tests, prescription of pharmacologic agents and treatments, non-pharmacologic therapies, and counseling patients.
Department: Employee and Community Health / Primary Care Internal Medicine
Basic Qualifications: Masters prepared graduate of an accredited school of nursing and Nurse Practitioner program. Current Minnesota RN license and certification or eligible to be certified as a FNP or ANP (certification must be obtained within 6 months of employment date.) Or graduate of accredited Physician Assistant program with current PA certification by NCCPA and registration as a PA by the Minnesota State Board of Medical Practice (or eligible). BLS certification required. DEA registration will be required. Please submit resume and 2-3 letters of reference.
Preferred Qualifications: Minimum of two years outpatient primary care experience preferred. The individual should be interested in working with patients with chronic disease, be comfortable with the use of technology, and have an interest in program development.
